You can report a loss on a T5008 or under the "Income" tab "Investments" > Capital Gains. Make sure that you have arranged with a brokerage to dispose of the stock as a loss can not be created unless the stock has sold. Most brokers have a procedure for this.
If the company you invested in was a Canadian Controlled Private Corporation then you may be able to claim an Allowable Business Investment Loss. Provide this information under the "Deductions" tab > "Losses" > Allowable Business Investment Loss.
