You report the amount from box 5 of the SSA-1099.  It still needs to be shown.

The part you paid for medicare is also entered from the SSA-1099 and this is deductible as a medical expense if you are able to itemize.

but what if I did not receive a 1099SSA and when I contacted SS they told me I did not have any benefits and no 1099 filed?
