Does Guaranteed Income Supplement have to be applied for every year?

As per CRA- Guaranteed Income Supplement – If you receive the guaranteed income supplement or allowance benefits under the old age security program, you can renew your benefit by filing your return by the filing deadline. If you choose not to file a return, you will have to complete a renewal form.

No need to re-apply for it. Since you already have it and you are filing your 2016 return it will automatically get calculated for you.
