I am claiming home expenses for a self proprietor based business but have moved during the year. Do I file two separate T2125 forms to do this?

Yes that is correct, separate T2125 entries will be required for the portion of the year at each address. You can do this from the T2125 Business Summary screen and selecting Enter New Business for X.
