The T5008 "Summary of dispositions" didn't come with an official looking slip with "box" numbers. Should we file a 5T008 form?

Money with withdrawn from an RRSP, received a T4RSP for that.

Also received a T5008 "summary of disposition" from the bank, but it doesn't seem to be an official T slip. It doesn't have BOX numbers.

Definitely only withdrew from the RRSP once in 2018 so I'm not sure if I still need to enter a 5T008 form?

You are required to enter whatever Tslips, or receipts received for RRSP or T4RSP unless otherwise advised not to.
