I am a retired MD, with no further business, but I had to write a legal letter about a previous patient in 2016, & charged $150 for this. Where do I enter it on my return

and can I claim expenses against this income?

You can claim this under occasional earnings. (Income>Employment Status>Other Employment Income Sources)

If you were are wishing to claim expenses against it, you would need to file it as self employed income and claim the income expenses on the business statement.
