My daughter is 21 & student. She has ADHD.She qualified for a disability grant through national student loans program.Do I declare her disability on her tax return?

I have not claimed her disability on her tax return previously but she was approved through the National Student Loan program.

The grant your daughter received is a "Canada Student Grant for Students with Disabilities." While the grant itself is a financial award, "declaring a disability" on a tax return refers to the Disability Tax Credit (DTC), which is a separate process.

Has a doctor ever filled out Form T2201 (Disability Tax Credit Certificate) for her, and has the CRA approved it?

Simply having a diagnosis or receiving a student grant does not imply she is "disabled" in the eyes of the CRA. To "declare" a disability and receive the Disability Tax Credit ($9,000 or more), she must have an approved Form T2201 on file with the Canadian Revenue Agency.
