I need to have a date of disposition on my T5008 forms. The forms I downloaded from CRA did not have them. Any ideas?

The standard T5008 slip from the CRA doesn't have a specific box for a "date of disposition" but instead, it uses Box 14 for the date of the securities transaction. You should enter the date the transaction occurred (sale, redemption, etc.) in that box.
