Somehow Turbotax has assigned the medical expenses to the spouse with lower income, where the resulting deduction is not needed. The federal one is assigned properly. Toggling the assignment changes the federal but not the provincial. I believe the first time it defaulted to my spouse, but it explicitly says you can change it later. But apparently not for the Quebec part.

We'll need you to clarify your question and to let us know if you are using TurboTax Online or Desktop.
Normally, when you apply medical expenses to one of the 2 spouses in a joint return (in your situation, the one with the higher income), the expenses will be applied to that same souse on both sides, federal AND provincial.
Make sure to review the medical expenses entries and review the medical expenses section. Also verify to apply it to the person where it's most advantageous.

Quebec is different as it has its own forms for everything.
When I select a spouse to assign the medical expenses to, it ONLY changes it on the Federal form. And there is no way to change the Quebec provincial selection. As it happens, Quebec makes you deduct 3% of both spouses income from the medical expenses, so changing spouses makes little difference. Unless of course the lower income spouse doesn't need the deductions. I think this is a bug, though I can't say that this overly impacted me this year.
