For you to claim a mental/physical impairment, you must have a Form T2201, Disability Tax Credit Certificate completed by a medical practitioner, this form must be approved by CRA.

 

The mental/physical impairment must meet some criteria outlined by CRA one of which is that the condition must last or expected to last at least one year, another is if the anxiety/depression restricts you in one or more of the following basic activities of daily living: (Speaking, hearing, walking, eliminating, feeding, dressing, performing the mental functions necessary for everyday life).
