I prepare15-20 tax returns (mostly simple) for my family and close friends. Can I use the Premier version and add on to the 12? If so, at what cost? Or should I buy the 20 return version?

In your case, for larger families our best solution is the TurboTax 20 returns . 

If you choose 12 returns and you try to add single return each time, you might end up paying more, specially if your close friends have specifics T-slips (investments, property sold etc...).
