How do I claim travel,meals and accomodations for medical appointments and surgeries

I travelled 3378 kms for specialist appointment and surgeries during the last year. I also spent 7  nights in Motels and 24 meals during these trips.2 trips from Golden B.C to Kamloops B.C. and return for cataract surgery.1 trip to Kelowna for heart checkup. I also made 6 day trips from Logan Lake to Kamloops for specialist appointments.

You should do up a working paper for the sake of convenience.  List each trip separately.  Example below:

Golden to Kamloops = 360 km x 2 return = 720.x $0.495 = $356.40

Meals expense lunch and dinner day 1 x 2 for patient and attendant = 17.x2=34.00 x 2 = 68.00

Day 2 3 meals each person = $51.00 per day x 2 =$102.00

Letter from the doctor stating the travel is required as services are not available locally, and that an attendant is required to assist with the travel.

Receipt for accommodations = Hotel Zanzibar = 1 night $150.00

1 day back home Lunch and dinner as above for 2 persons = 68.00

  You would not claim meals for the Logan Lake trip.  Your attendant can claim meals while waiting for you when you are hospitalized.
