My medical expenses were over 16,000. Why was the refundable medical expense supplement 0?

I was expecting the refundable medical expense supplement  to be 1203 (line item 452 on fed tax return).

Other than the medical expenses themselves, there are other eligibility requirements for the refundable credit.
