Im really confused. What are the Source and Institution names for a T5 and T5008

I am very sorry, However;  I am unclear on your question.  Are you looking at what company would send you a T5?

The links are useful but more oriented to issuers of T5s and T5008s.

On a T5, the Source is where and in what form you have received investment income.
Examples , Tangerine savings interest, or TD Bank dividends. If you read the back of your T5, all this is explained.

On a T5008, The Institution is the bank or brokerage that has issued the slip.
