Employee paid premium is the amount in Box 85 of your T4. Unlesss in was not reported in a T-slip and you have a letter from your employer with this amount.
If you pay extra health insurance premiums such as travel insurance, you can claim them.
Any other eligible medical expense such amounts not covered by your insurance on prescribed medications, physiotherapy, chiropractors, etc. Please refer to the CRA link below to check the medical expenses allowed for 2019:
