    When it asks ' What was your self-employmen income- is this my gross revenue for the year? it has a subtext of 'professional fees' so I am confused.

    Yes, this would be where you enter your gross income. Don't include GST/HST if you are registered to collect GST/HST - you'll submit that info when you do your GST/HST return.
