    Do I need to report both foreign income on line 12100 and T5 for dividends reported on US form 1042-S and Canadian T5?

    The amounts on Form 1042-S and T5 are not the same.

    Do I need to report both foreign income on line 12100 and T5 for dividends reported on US form 1042-S and Canadian T5?

    Are the amounts on the two forms different because one is in USD and the other is CAD?
