Where can I complete the General Conditions of Employment (TP-64.3) form?

The form has been completed and i still have the message preventing me from submitting my provincial income tax ! very frustrating

Ensure you fill all fields on form TP64.3. In field 3.1, enter "yes" only if you have employment expenses. Otherwise, enter "no".
