I work part time as a freelance graphic designer . I earned more than $30k this year. Do i use form T2125 or just file as other income? I have expenses , I did not collect tax on the earnings . Should I be calculating the HST on each of the flat rate jobs i did?
Yes, this is the correct form and is available using TurboTax Online Home & Business. You will be guided through this form when you indicate you had self-employment income.
More information relating to the T2125 is available here:

Once you earn $30,000 in one year, or in any one reporting period, you must register for GST and collect it on all sales. You will then also be able to claim back the GST on all of your expenses. It is a separate reporting process, not done through your income tax.
You can contact CRA for details at 1-800-959-5525.
