I didn't receive one of my slips and I've just realized it.

If you are registered for the My Account service on the Canada Revenue Agency website, you can log into your account and retrieve the information about your T5 slip.

If you are not registered for the My Account service, you can contact the Canada Revenue Agency at 1 800 959 8281 and they can provide you with the information over the phone.
