How to report eligible dividends and capital gains from Canada without T5 slip due to foreign broker

Received Canadian eligible dividends with a non-Canadian securities broker, as such, there is no readily available T5 slips to report the investment income. 

 

At the broker end, they treat me as non-resident of Canada, so there was  a 15% withholding tax paid. 

 

Eligible dividends: $200

Capital gains dividend: $100

Withholding tax paid: $45

 

Without a T5 slip, could you please advise how to reflect the above information in Turbotax?  Tried but still doesn't seem to get it right.

Here is the Canada Revenue Agency (CRA) link to the T5 pdf to be reviewed so you you can enter your amounts. You should contact the CRA for clarification, about the other information you gave. You can reach them at 1-800-959-8281.
