Why are combined medical expenses not affecting our return? Expenses are higher than the 3% calculation of the lower income person.

As per our TurboTax FAQ: Non-Refundable Tax Credits: A non-refundable tax credit reduces the amount of tax you pay on your taxable income. You will not get money back from a non-refundable tax credit, but you can use it to offset how much you will pay.

You may not need the medical expense to lower your taxable income.
