Rl-1 line 381 what is put there? And what letter does that fall under when I am completing my relevé 1

Line 381 is medical expenses and from the RL-1 it would be under box J.  You can also claim medical expenses that you paid for which was not reimbursed to you by your health/medical insurance plan.

Do I put it on both rl-1 and T4? If yes, where do I put it on t4?

should be also included in box 85 for T4, boxJ for releve 1.  You can put it according to the slips as Quebec and federal may be different amounts.
