If I have to travel at least 40km to obtain medical service and cra list these requirements listed below.

You took a reasonably direct travelling route.

What does substantially equivalent medical service mean? Does it mean it only applies when service does not exist at my area and only at least 40km away? Also when I claim the amount could I claim both ways or is it only one way?
